Who We Are

Building healthier
communities together

"To educate communities, support patients, and help people take charge of their health — one conversation at a time."

GROW – Grace Reaches On Wheels was founded in Dar es Salaam, Tanzania, to help underserved communities understand and manage non-communicable diseases (NCDs). We bring together health education, technology, and hands-on community work.

Since launching in May 2026, GROW has combined culturally grounded education with AI-powered tools to reach communities across Tanzania and beyond. We started with digital outreach, and we're growing into mobile health services and stronger local partnerships.

We run as a hybrid initiative — a non-profit arm focused on awareness and patient support, alongside a sustainable services arm that funds our long-term work through mobile health and pharmacy services.

Core health topics we educate on

❤️

Cardiovascular Disease

Heart disease and stroke are the leading cause of NCD deaths globally — yet most cases are preventable through early intervention and lifestyle change.

  • Monitor blood pressure and cholesterol regularly
  • Reduce salt, saturated fats, and ultra-processed foods in your diet
  • Aim for at least 150 minutes of moderate physical activity weekly
  • Avoid all forms of tobacco and limit alcohol intake
  • Manage chronic stress — it significantly impacts cardiovascular health
🩺

Diabetes

Type 2 diabetes is among the fastest-growing NCDs in sub-Saharan Africa. It is largely preventable and manageable with the right knowledge and tools.

  • Maintain a healthy body weight through balanced nutrition and activity
  • Limit refined carbohydrates, sugary beverages, and processed snacks
  • Test blood glucose regularly, especially with a family history of diabetes
  • Symptoms: excessive thirst, frequent urination, fatigue, blurred vision
  • Unmanaged diabetes leads to kidney failure, blindness, and amputation
🫁

Respiratory Disease

Chronic respiratory conditions such as asthma and COPD are exacerbated by indoor pollution, tobacco, and environmental factors prevalent in our region.

  • Eliminate tobacco use — the single most important preventive step
  • Transition to cleaner cooking fuels and improve household ventilation
  • Seek medical attention early for persistent cough, breathlessness, or wheezing
  • Annual lung function checks are recommended for high-risk individuals
🧠

Mental Health

Mental health is an integral and often overlooked dimension of non-communicable disease. Chronic illness significantly amplifies psychological burden.

  • Seek professional or peer support — early help leads to better outcomes
  • Strong community ties and social belonging are protective factors
  • Regular physical activity and adequate sleep substantially improve mental health
  • NCD patients benefit greatly from integrated psychosocial support
🌿

Preventive Lifestyle

The majority of NCDs share a common set of modifiable risk factors. Sustained lifestyle change at the community level offers the highest return on health investment.

  • Adopt a diet rich in whole foods — vegetables, legumes, fruits, and grains
  • Stay hydrated and reduce dependence on sugary or processed drinks
  • Eliminate tobacco in all forms — cigarettes, shisha, and smokeless products
  • Prioritise consistent sleep and evidence-based stress management
  • Schedule annual preventive health check-ups without waiting for symptoms
